CCD is a multi-service registered charitable organization providing a broad range of support services for individuals over the age of 16 and their caregivers.
Community Care Durham’s Transportation services provide a great solution when you are unable to use public transportation because of needs related to aging, physical and/or mental health and when friends or relatives are not available to help you.
Are you in need of a ride?
Rides, primarily to medical appointments, shopping, errands and appointments and to access CCD programs, are provided by CCD staff using CCD vehicles or volunteers using their own vehicles.
What Types of Transportation Services are Available
Due to additional expenses caused by the pandemic, we’ve had to make the difficult decision to raise our fees, effective April 1.
Varies by type of transportation service required. A family member or friend is welcome to ride along with you at no additional cost in order to accompany you to your destination.
Where Is Transportation Offered
Rides can take place within Durham Region, to neighbouring communities or across the Greater Toronto Area.
How Can I Book a Transportation Service
All rides, including those provided by Volunteer Drivers, must be booked through Community Care Durham. Community Care Durham assumes no liability or responsibility for services undertaken or arranged without the knowledge or approval of the local office.
To register for the Transportation service, please call 1-888-255-6680 or complete the online referral form found at Request Service.

CCD’s transportation services help me remain independent and connected with the community.
Access to Primary Care is an excellent service, the PSWs are always so willing to help me with my walker and ensure I am able to get to my Doctor’s appointment on time.
The program is so helpful to us as a family. We are not always able to take mom to her appointments and we know that we can rely on the Transportation program to get mom safely to her where she needs to go.
