Community Care Durham refreshed its Strategic Plan in Spring 2021. No less committed to our Mission and Vision, this Strategic Plan builds on the organization’s previous Strategic Plans and represents a forward looking, innovative future for Community Care Durham (CCD). It is fueled by a commitment to be leaders supporting residents across Durham Region to achieve their personal health and wellness goals.
