The Community Food Box Program is an affordable approach to safely accessing healthy, nutritious food with options to cater to our clients’ needs.
Our Fresh Food Box contains fresh produce, dairy products, and freshly baked bread.
$35.50 per box. Free contactless delivery every Friday.
Orders must be placed by Wednesday a week before delivery.
CCD’s Meals on Wheels service provides clients and their caregivers with the security of knowing they can live independently in their own home through the delivery of affordable, tasty and nutritious Hot and Frozen meals.
Accessible Vans (AV) transportation service provides wheelchair accessible vans driven by CCD staff to transport clients with mobility issues. Community Care Durham’s Assisted Living Services (ALS) and Supportive Housing (SH) services address the needs of high-risk seniors residing at home.
Community Care Durham’s Home to Stay Program is a full fee for service program available to clients requiring the skills of an Experienced Caregiver for routine activities of daily living and instrumental activities of daily living.
Community Care Durham’s Home at Last (HAL) service supports adults who require assistance to settle back at home after a hospital stay.
CCD’s Exercise and Falls Prevention Program is a community-based initiative aimed at meeting the exercise needs of Older Adults in the Durham Region.
CCD’s Foot Care program is an important Health and Wellness service available to people who need assistance with basic foot care due to loss of vision, mobility issues or diabetes or when friends or relatives are not available to help.
Clients are matched with one of CCD’s Friendly Visiting volunteers and receive much needed social interaction, so they can maintain their independence and quality of life while safely remaining in their home.
This CCD service is ideal for anyone who is not able to leave their own home, socially isolated or lacking caregiver support.
Community Care Durham’s Transportation services provide a great solution when you are unable to use public transportation because of needs related to aging, physical and/or mental health and when friends or relatives are not available to help you.
CCD’s Personal Support Workers partner with caregivers to ensure their loved ones are safely cared for while caregivers take time for their own activities, helping to prevent caregiver burnout and illness.
COPE (Creating Opportunities for Personal Excellence) is a unique community-based mental health service provided by Community Care Durham that offers support to adults aged 16 and older, with emotional and/or mental health concerns.
