Community Care Durham (CCD) is a multi-service registered charitable organization providing a broad range of community support services for adults and their caregivers who have needs related to aging, physical and/or mental health. We assist our clients with services that support their independence, health and quality of life. In doing so, we help them remain in their own homes – where they want to live.

Partners & Supporters
CCD’s main funding support is from the Province of Ontario’s Ministry of Health & Long-Term Care through the Central East Local Health Integration Network (CE LHIN). In addition, funding support is received from the United Way of Durham Region, area Municipalities and our generous donors.
Full Services Offered
Some of our services (i.e., Friendly Visiting, Telephone Reassurance, Mental Health support, Help getting home from Hospital, Assisted Living, assisted Transportation etc.) are offered free of charge. Other services (like Meals on Wheels, Luncheon Out, Foot Care, Personal Support, Day Program, Transportation, Exercise & Falls Prevention classes etc.) have a service fee in order to assist with the total cost of service delivery.

Our offices are located throughout Durham Region in the communities of Ajax, Brock, Clarington, Oshawa, Pickering, Scugog, Uxbridge and Whitby.
