Do you have an Ontario Driver’s “G” or “G2” License?:(Required)

Are you willing to drive clients in your vehicle?:(Required)

Employment Experience
Are you, or were you an employee of Community Care Durham?:(Required)
Have you worked a minimum of 600 hours in duration, including in class and practicum experience, or other acceptable experience (e.g., RN or RPN, or 2nd year nursing student?):(Required)
