For 45 years Community Care Durham (CCD) has provided community supports helping Durham Region residents live safely and independently wherever they call home. We’ve played a vital role in supporting our partners in the healthcare system, preventing unnecessary hospital visits by clients through regular Personal Support Worker supports and services. But our healthcare system has reached a breaking point. Nursing shortages have led to temporary emergency room (ER) closures across the province and here in Durham Region.

That is why the support CCD provides hospitals has never been more important than now. As we celebrate 45 years of supporting residents in Durham Region, CCD is launching its No Place Like Home capital campaign. Our goal is to raise $3M to transform our location at 20 Sunray Street into a Health & Wellness Community Hub – a “one stop shop” for residents of Durham Region to access health and wellness services delivered by integrated care teams. This redesign will bring together Adult Day Services, food security, a new community kitchen, supports for caregivers and those with mental health needs, nursing services and so much more. Our goal is to build this initial Health & Wellness Community Hub in Whitby first, with the goal to expand to our other hubs throughout Durham and create long-lasting social impact.

What will this new Health & Wellness Hub mean for the Community?
