As a trusted community partner, Community Care Durham (CCD) is committed to supporting Durham residents living with frailty, their caregivers, as well as those seeking to maintain their wellbeing. Our 9,000 clients range from 16 to 106 years of age and have a wide variety of needs, but they all share a common desire: to live an optimal and independent life in their homes, and in the community.

Ontarians are now making choices about their elected leaders who will shape our future. We urge voters and candidates to prioritize enhancing community-based health and wellness supports. Not only are community solutions cost-effective, but they are what Ontarians want and deserve.

Fund OneConnect:
An Innovative Community-Based Approach for Older Adults Living with Frailty

Services for adults living with both chronic health and social challenges are often fragmented, leaving clients and their families exhausted as they try to navigate a complex and disjointed system. All too often, people are falling through the cracks and are seen as “transactions” by different parts by a system siloed by bureaucratic barriers. As a result, families are unsupported and living on the breaking point.

In Durham Region nearly 27,000 older adults are living with frailty. Frailty leads to increased hospitalization and lost independence. Over 2,300 Durham residents are currently on the waitlist for long-term care, with an average wait time of 578 days. A third of them are living alone. Demand for long-term care is expected to rise by 38% over the next decade.

We need a better solution. Solutions that put people first and are fiscally sustainable.

OneConnect provides a cost-effective solution to help older adults age in place while maintaining their independence and quality of life. Our proposed model will provide clients and their caregivers a comprehensive range of health and social supports through a single integrated program. Instead of chasing services, OneConnect wraps around the client and moves supports seamlessly as their needs change.

Not only will OneConnect reduce hospitalizations and prevent long-term care admissions, it will cost Ontario’s taxpayers less than half of what it costs to provide care in long-term care. Rather than building another new long-term care home in Durham, we are asking government to fund “long-term care at home” for 150 local residents.

The “made in Durham” solution will put our region at the forefront of healthy aging innovation in Ontario. Now is the time for us to lead the way in making this the community we want to age in.

Expand Adult Day Programs
to Meet Urgent Demand

Adult Day Programs provide vital support for adults with needs related to aging, physical, and cognitive health. These programs offer socialization and structured activities in a safe and supportive environment, while also providing much-needed respite for caregivers.

CCD is seeking to support to expand Adult Day Programs, notably in Ajax where there is no publicly supported program. Demand for Adult Day Programs is growing. Not only are there new referrals on a long wait list, existing clients and their caregivers are unable to receive more days of support.

Adult Day Programs reduce caregiver burnout, improve overall well-being, prevent hospitalizations, and delay the need for long-term care. Most of all, it’s a place where people can come together – whatever their physical or cognitive challenges – breaking down the perils of loneliness and isolation by discovering a community of support and friendship, and experience personal joy and renewed purpose.

Advance Technology to
Enhance Community Care

Ontarians believe that connected care is better care. However, Ontario is not keeping pace with technological advancements in the Community Support Sector, limiting the ability to provide seamless and person-centred services.

Despite funding challenges, CCD has been investing in technology to enhance client care while improving operational efficiency. But provincial funding is almost entirely absent, and what incremental funding we receive isn’t keeping pace. In the past years, our technology costs have increased 7% annually and will continue to rise 4 to 5% in the years ahead.

We urge the next provincial government to support investments in technology to fully unlock the potential of community-based solutions.

Community Care Durham calls on all candidates to commit to funding and policy solutions that strengthen community-based health and wellness supports. Investing in these priorities will ensure that Durham residents receive the support they need to live and age, safely and independently.

Together, we can build a future where older adults thrive, caregivers feel supported, and public funds are maximized for lasting impact.
