Home at Last (HAL) Workers -- Casual (Elect-to-Work)
Personal Support Workers
Qualifications required:

Personal Support Worker certificate, or acceptable equivalent (e.g., RPN) and experience in the community setting
• Maintenance of a class G Ontario driving licence and clean Drivers Abstract
• Use of a licensed vehicle and proof of $1,000,000 public liability insurance and the appropriate business-use coverage as required by the Worker’s auto insurance carrier
• Satisfactory criminal reference check
• Sensitivity to the needs of the frail elderly and persons with disabilities
• Physical ability sufficient to assist with client transfers and activities of daily living including toileting
• Ability and willingness to accept assignments on short notice and drive within Durham Region

Compensation: $17.14 - $20.49 per hour plus 4% vacation pay

The “Home At Last” (HAL) Program has been established to assist with timely and safe discharge of hospital patients to their homes in situations where their family or caregiver is not available to transport them home when they are due to be discharged.

The HAL Worker provides transition assistance to clients, including transportation from hospital to home. It is pre-arranged if the Worker is to stay with the client until family arrives or if the Worker leaves when the client is settled safely and comfortably. (The service is not designed to be an overnight service.The HAL Worker provides client support at the hospital, in transition and at the home in accordance with Community Care Durham’s policies and procedures, and communicates with the HAL Care Coordinator and/or supervisor as directed
