Compensation: $18.19 - $21.74 per hour; health and pension benefits when eligible; 3 weeks vacation pro-rated to hours of work, and paid (stat) holidays as per policy.

Service Coordinator, Assisted Living Services
Scheduled Part Time (21 hr/wk)

Implementing and evaluating services, in consultation with the Program Manager, to meet client needs |
| • |
Scheduling/liaison with staff to coordinate services |
| • |
Completing client assessments and follow-up as delegated; ensuring accurate documentation |
| • |
Liaising with various organizations and individuals in the coordination of service |
| • |
Creating and maintaining spreadsheets (Excel) for statistical data, billings, etc. |
| • |
Reconciliation of time sheets for payroll input |
| • |
Promoting Community Care Durham's services; working within the team to support all services; assisting with Program/Community Care Durham initiatives. |

Relevant post-secondary preparation in human services, and experience in administration and scheduling, or an equivalent combination of education and experience |
| • |
Demonstrated communication and organization skills, initiative, flexibility and accountability for delivering results within deadlines |
| • |
Good knowledge of community support services, scheduling and human resources management |
| • |
Intermediate MS Office skills. Experience with database programs would be an asset |

Satisfactory police record check with vulnerable sector screening |
| • |
Licensed vehicle and the ability to travel within Durham Region |

Sensitivity to the needs of the elderly and persons with disabilities
